Thank you for trying AFD online.  The following information is provided to help you successfully submit an online consultation to Dr. Lawrence.

     Services found within the blue bar at the top of the web-page, like VIRTUAL HOUSE CALL or ASK A DOC, are password-protected by a secure portal.  Using these services is like stepping into a private exam room where only you and the doctor have access to the information.

HOW TO SUBMIT AN ONLINE CONSULTATION:

STEP 1: Create an account

     First click CREATE AN ACCOUNT.  Fill in the required information to establish a secure account with AFD.  Review and acknowlege receipt of AFD's privacy policies.

     NOTE: Some users have had trouble submitting a consult right after creating an account.  PLEASE LOG OFF THE SYSTEM AND LOG IN AGAIN before going to Step 2.

STEP 2: Submit an online consultation

     VIRTUAL HOUSE CALL.  Enter your account name and password. Choose the symptom from the list that most closely corresponds to your problem.  You will then answer a series of questions followed by an opportunity to enter your vital signs and attach additional information including documents, photos, or video clips.  Select your preferred pharmacy from the list.  Your preferred pharmacy can be added if you do not see yours in the list.  See SERVICES for further information on use of this feature.

     ASK A DOCTOR.  Enter your account name and password.  Type or paste your question in the field.  You may then attach any documents, photos, or video relating to your question.  See SERVICES for further information on use of this feature.

     PRESCRIPTION RENEWAL.  Enter your account name and password.  Select your preferred pharmacy.  Your preferred pharmacy can be added if you do not see yours in the list.  Include as much detail as possible regarding the medication name, dosage, amount needed, and indications for using the medication.  See SERVICES for further information on Prescription renewals.

STEP 3: Reviewing your response.

    You will be sent an e-mail notification as soon as Dr. Lawrence responds to your request.  To review your message, simply follow the instructions included in the email and sign-in to the secure portal.  Before completing the consultation, you can send a response back to Dr. Lawrence or request further information. 

STEP 4: Payment

     You will be asked for your credit card information to initiate a consultation.  Your card will ONLY BE CHARGED at the completion of the consultation.  Feel free to ask Dr. Lawrence about other payment options.
